Thank you for taking the time to participate in the Performance Culture Assessment survey. The goal of this survey is to determine how we are doing as an organization and to identify opportunities for improvement. As Catholic Health Initiatives continues to grow its health ministry, it is important to align actions across the system to develop the workforce culture necessary to effectively implement strategic and operational priorities. An important component of the workforce culture is keeping in touch with the opinions of our employees in an open, honest and specific way.

Important note about confidentiality: Your unique password is the first letter of your first name plus the first letter of your last name plus your Lawson ID number or the ID number assigned to you. This unique password will ensure you are directed to the correct survey for your facility and that your responses are linked to your work group. Please be assured that only work groups with five or more respondents can be reported separately. No one at CHI or at the facilities will be able to view responses provided by individual employees.

Your ID number is the first letter of both your first and last names followed by your Employee ID. For example: If your name is John Smith and your Employee ID is 12345 your password will be JS followed by your Employee ID or JS12345.
